The Onyx & Breezy Foundation provides funding and support to

  • Spay and neuter programs
  • Rescue of animals from kill shelters
  • Food, medicine and supplies
  • Cancer Research
  • Medicine and equipment for military and police canines
  • Equipment for medical facilities
  • Puppy mill rescues and disaster relief
  • Shelters, rescues, foster groups and sanctuaries
  • Pets of individuals where medical hardship is present
  • Helping the dogs of veterans suffering from PTSD
  • Other endeavors that benefit the welfare of animals

Grant Requirements

All grant applications must be returned with the following to be presented for consideration:*

  • Financials

                 Individuals 
                   -Tax Return (Copies only)
                 Non-Profit Organizations
                   - Last Audited Financials
                   - Current Balance Sheet & Income Statement (Not older than 60 days)

  • Diagnosis & Prognosis (if applicable)
  • Estimate of cost
  • Care Credit status (individuals must apply for care credit and submit evidence of either denial or approval with available balance)
  • Telephone number that accepts calls from a "blocked' telephone number.

(Grant Packages MUST be sent all together and complete at the same time. Incomplete or piecemeal packages will NOT be reviewed)

PLEASE NOTE:

  • Applications over ten pages must be sent as hard copies through the mail to the address above
  • Incomplete grant requests will only be held for 15 days
  • Grant requests which have been approved are valid for 30 days only

(Please note that we do not fund requests for medical care previously paid by applicant.)
